Premixing Request Form

In order to premix large quantities of mixed drinks, you must have your recipe approved by the Commission.  To submit a recipe for approval, first enter your Mixed Beverage permit number below:

* Your mixed beverage permit number is printed on your permit. Include ALL numbers and any letters before or after the numbers.
